Functional Neuro Surgery

Intra thecal durg delivery system : (For intractable pain and spasticity)

For chronic pain intrathecal durg delivery system delivers morphine into the spinal fluid. This has the advantage of very low dose of drug requirement with no side effects. It is useful in intractable cancer pain, failed back surgery syndrome, painful neuropathies. Intrathecal baclofen pump is indicated in severe spasticity. due to cerebral palsy, Spinal cord injury, head injury, multiple sclerosis etc.

Spinal cord stimulation :

In painful diabetic neuropathy, post amputation pain, failed back surgery syndrome, Refractory angina and other chronic painful conditions

Deep brain stimulation : (DBS)

This involves stimulating nerve complexes is deeper part of brain called nucleus with micro electrodes. Different areas of brain are stimulated in different diseases. Advanced parkinsonism, Various types of Dystonias are common conditions usually treated by DBS. DBS is also useful in various psychiatric conditions like intractable obsessive compulsive neurosis, intractable depression, etc. and for addiction (alcohol, nicotine & hartin)

Endoscopic Neuro surgery

Neuro endoscopes are introduced into the brain to perform surgeries like tumour biopsy excision of colloid cyst, fenestration of cyst and third ventriculostomy. Endonasal endoscopic surgery is performent for pituitary tumors and chordomas

Surgery for epilepsy

Epilepsy is a neurologic condition characterized by seizures. In India, the number of people with epilepsy in India will be around 5.5 million. About 500000 new cases are added every year.

Epilepsy is treated with medicines and effective in most cases. Most patients do well with medicines but upto 35% of patients become resistant to even multiple drugs. Moreover there are two other problems: one is the cost of the drugs and the second is the side effects due to the drugs. All drugs cause side effects like rashes, thickening of jaws, bone and liver damage and blood clotting abnormalities and congenital abnormalities. In fact more than 25% of patients have stopped treatment because of side effects.

The Main treatment of epilepsy is always medical, but is medically unresponsive cases and those who are intolerant to medical therapy surgery is an option. The Surgical options include lesionectomy, temporal lobectomy, corpus callosotomy, hemispherectomy and vagal nerve stimulation.

Surgery for epilepsy is well established. Studies have shown that surgical candidates have a much higher chance of attaining freedom from epilepsy (58% vs 8% for medical treatment). It has been well demonstrated world over, including in India, that epilepsy surgery is safe and is associated with very low mortality and morbidity (2%). The presence of a "concordant" epileptogenic focus in a patient with drug resistant epilepsy would form an indication for "respective" epilepsy surgery. About 70 -85% of patients become seizure free after epilepsy surgery.
